Tablet Contract and Enrollment Paperwork

Dear Parents,

Please see our enrollment paperwork for 2020-21.  To complete, just click on the link for the enrollment form.  Completed documents will be submitted electronically.

Tablet Contract

Please sign the appropriate contract according to your child’s grade level. Contracts require a signature from a parent and the student.

Please complete the tablet contract before we can distribute it.  

The remaining enrollment paperwork needs to be completed by Monday, August 17, 2020.

If you completed a lunch application at LAUSD you still need to fill out a lunch application with HTLA.
